John F. (Jack) Perhac, age 74, of Hayes, died at West Point Convalescent Center on Nov. 25, 2018.

Jack was retired from Home Beneficial Life Insurance Company.

He is survived by his wife Evelyn; his son, Stephen; daughter-in-law, Jessica, and granddaughters, Genevieve and Sadie. His sister Janie lives in Pennsylvania, along with a niece and two nephews.

As per Jack’s wishes, services will be private.
